I hereby grant permission for my child to join the DC Speed Track Club located in Washington, D.C. Further, I grant D.C. Speed permission to release data on my athlete to the D.C. Department of Parks and Recreation (DPR), which may include their name, address, and registration status with the DC Speed.

I waive any claims against the DC Speed Track Club, the DC Speed Track & Field Booster Club, the D.C. Department of Parks and Recreation, the D.C. Government, and/or any of its representatives, coaches and/or staff in regards to any harm sustained by my child due to participation in any Club activities unless such harm is due to a willful act or gross negligence on the part of the involved party or parties against which the claim is made. Further, I represent that I am fully authorized to waive all rights to claims against the above named entities because I am legally recognized to do so on behalf of the above named athlete.

I grant signature authority to all coaches and Club officers for the purpose of registering my child for participation or for signing any track and field-related correspondence during the season. I grant D.C. Speed permission to use photos and/or videos taken of my child and myself for advertising purposes and for the furtherance of the mission of the organization. Further, I give the coaching staff of DC Speed Track Club permission and authority to be the legal guardians of my registered child, in my absence, at track practices and meets.

I acknowledge that I am aware that the registration process is not complete until I submit the remaining forms in the registration packet and pay the applicable registration fees in full.
